Requirement for Working Card

1. Complete Part I of the Application and the entire Physical Fitness Certification form.  

2. Make sure the application is signed (in Part 1) by parent/guardian.

3. Visit the school Nurse with completed forms for verification of  Physical Fitness Certification. If you do not have a current physical on file you will need to obtain a physical from your doctor and have the certification signed, dated and stamped by your doctor. 

4. Return the completed application and physical form to the counseling center form processing.

5. The student must be available to sign the card in person.

**** During the summer months and when class is not in session, you must present a signed, dated and stamped Physical Fitness Certification form (AT-16 linked above) from a physician.

*  NOTE: The working card is valid for two years and may be used for an unlimited number of successive job placements. Your employer is required to return the card to you upon termination of your employment.
